My orientation around this case:

1.What could be the possible cause if his anemia?

Ans: possibily vit - B12 deficiency

2.Why vit- B12 deficiency and not some other cause? 

Ans: 1)his peripheral smear showed few macrocytes.  2) patient consumes low non-veg diet. 3) his serum iron and serum ferritin level was in normal range. 4) his vit-B12 was lower than normal range.

My orientation around this case: 

1.What could be the reason for he being suspected for Parkinson's? 

Ans:Due to presence of tremors, slowness of movement, loss of balance. 

2.cause of his cellulitis ? 

Ans: injury due to reccurent fall. 

3. Cause of recurrent falls? 

Ans. Postural instability due to increased motor impairment. 

During the disscussion of the case in the clinical lecture hall:

1. What is micrographia ?

Ans: An abnormally small hand writing that is difficult to read. 

2. Can an injury to head cause Parkinson's? 

Ans: Head injuries can increase a person's risk of developing Parkinson's disease.

My learning experience :

During ICU duty 

Have performed insertion of ryles tube to a 65 year-old female with h/o stroke in ICU bed no 3 .

Have taken samples for ABG for a 55-year-old male with ARDS in ICU bed no 1 .

Have performed insertion of foley's catheter for the same patient. 

Learned to do CPR 

Have assisted in performing central line insertions. 

During nephrology duty

Have assisted in performing central line insertions. 

Collected venous sample for blood investigations. 

During casuality duty

Have given CPR to an old patient came with sob. 

Have performed cannula insertion. 

During ward duty

Have taken venous samples for routine investigations .
